Thurston Housing Adult Care Coordinator - $1,000 Sign-On Bonus!

Provide care coordination services to residential clients diagnosed with chronic and persistent mental illness, emotional disorders, and/or co-occurring disorders. Assist clients so that they may maintain stable and viable living standards in their homes and enhance their quality of life in the community. The Thurston Housing Adult Care Coordinator will work closely with clients to develop tailored treatment plans for the purpose of maintaining appropriate levels of care needed for personal hygiene, housekeeping, transportation needs, meaningful activities, emotional support, social support, medical support, and medication compliance.

Compensation: $23.88- $31.75 per hour depending on experience and qualifications.

QUALIFICATIONS:

  • Bachelor's degree in psychology, social work or closely related field.
  • Two years of experience with priority population preferred.
  • Previous experience working as a member of a multidisciplinary treatment team and working collaboratively with other service providers preferred.
  • Strong organizational skills to include use of EHR software or equivalent for scheduling, time management and all clinical documentation.
  • Have a valid driver's license and auto insurance.
  • While no longer required, as a health care provider BHR highly recommends applicants and employees are fully vaccinated against Covid.
