This member of the
multidisciplinary team has responsibility to carry out service coordination,
rehabilitation, and support functions for PACT clients. In addition, the Mental Health Professional
on the PACT Team provides assessment services, specialized group and individual
psychoeducation, and treatment for clients who have been diagnosed
with mental health and/or co-occurring mental health and substance use
Compensation: $21.04- $29.02 per hour depending on experience and qualifications.

RESPONSIBLE TO: PACT Team Leader and Outpatient Clinical Director
- Master's Degree in Human Services field
- 2 years post graduate experience
providing mental health services for clients with serious and persistent
mental illness preferred.
- Strong knowledge of chemical dependency
and mental health diagnosis and treatment.
- Must have a strong
commitment to the right and ability of each person with a severe and
persistent mental illness to live in normal community residences; work in
market jobs; and have access to helpful, adequate, competent, and
continuous supports and services.
- Skills and competence
to establish supportive trusting relationships with persons with severe
and persistent mental illnesses and respect for client rights and personal
preferences in treatment are essential.
- Must have a valid
driver's license for the state in which the program operates.
- Must be proficient in using evidenced
- Must have strong oral
and written communication skills.
- Must have the ability
to work as a team and individually.
- Must pass a criminal
background and Inspector General check.
TYPICAL DUTIES & RESPONSIBILITIES:
- Function as part of a multidisciplinary
team serving clients with persistent mental illness and/or co-occurring
substance use disorders.
- Be able to create person centered
treatment plans in a team setting with the client, and collaterals/family
members as available
- Provide ongoing
assessment of clients' mental illness symptoms and clients' response to
treatment. Make appropriate changes
in treatment plans to ensure immediate and appropriate interventions are
provided in response to changes in mental status or behavior, which put
clients at risk (e.g., suicidality).
- Provide direct clinical
services, utilizing evidenced base practices, to clients including
individual supportive therapy and psychotherapy to clients on an
individual, group, and family basis in the office and in community
settings to teach behavioral symptom-management techniques to alleviate
and manage symptoms not reduced by medication and to promote personal
growth and development by assisting clients to adapt to and cope with
internal and external stresses.
- Provide case management
services and attend outside appointments with clients, as needed.
- Meet with clients in
inpatient mental health hospitals when clients are hospitalized.
- Coordinate with outside agencies to
provide the best continuity of care.
- Be able to assist
clients with creating crisis plans
- Work with Designated Crisis
Responders as needed
- Ability to work
independently during rotating weekend shifts and when in charge of the
- Monitor clients that
are on a Least Restrictive Alternative or Conditional Release
- Assist clients with maintaining housing
in the least restrictive community-based setting available. This may
include helping the client learn how to do household tasks
- Deliver medications in the community to
clients on a rotating basis with other PACT staff.
- Schedule an average of 5-6 hours of
client service per day.
- Conduct mental health assessments as
part of the team comprehensive assessments that are part of the PACT model
- Develop and implement individualized
treatment plans that address client strengths, recovery, and mental
health/substance use symptoms.
- Maintain client records in accordance
with BHR standards.
- Adhere to all state and federal
- Write timely and accurate reports to the
courts and other involved parties as required.
- Participate in staff meetings, clinical
supervision, and scheduled training as required.
