PACT Psychosocial Rehabilitation Specialist - $1,000 Sign-On Bonus!

This member of the Program for Assertive Community Treatment (PACT) multidisciplinary team has the responsibility to carry out rehabilitation and support functions and assist in treatment with specific focus on identifying and supporting issues of recovery and ADLs.  In addition, they will provide case management services to individuals with severe and persistent mental health disorders who are most at risk of hospitalization, incarceration, and/or homelessness due to their mental health symptoms.  This includes assisting clients in maintaining stable housing, increasing independence, monitoring and assisting in medication compliance, assisting with coordinating medical care, and assisting with coordination between other service providers. 

Compensation: $23.38- $31.25 per hour depending on experience and qualifications.

QUALIFICATIONS:

  • Bachelor's degree in psychology, social work or closely related field.
  • A minimum of two years' experience working with individuals with severe and persistent mental health disorders preferred.
  • Previous experience working as a member of a multidisciplinary treatment team and working collaboratively with other service providers preferred.
  • Strong organizational skills to include use of Electronic Health Record software or equivalent for scheduling, time management and all clinical documentation.
  • Have a valid driver's license and auto insurance.
  • While no longer required, as a health care provider BHR highly recommends applicants and employees are fully vaccinated against Covid.
